If you want to travel from Kampar to Penang Island (George Town) by train there are a few KTM Electric Train Services (ETS) Gold departures a day to Butterworth Railway Station on the mainland, 15 minutes ferry ride away from the island.

Train EG 9322 (ETS Gold) departs Kampar at 19:17 - Arrives Butterworth at 21:30.
Train EG 9422 (ETS Gold) departs Kampar at 20:26 - Arrives Butterworth at 22:39.
Train EG 9122 (ETS Gold) departs Kampar at 22:11 - Arrives Butterworth at 00:22.
Train EG 9124 (ETS Gold) departs Kampar at 00:48 - Arrives Butterworth at 02:59.
All these ETS trains operate daily.
Travel by train from Kampar to Penang is not the cheapest way (buses are cheaper) and also there is a problem with the timings of your arrival in Butterworth Station, as the later trains will miss the last ferry the leaves from Butterworth at around midnight.
So if you want to take the train, take an earlier one or take a train to Batu Gajah or Ipoh first and take one of the earlier Platinum Services from there.

You should always make an advance booking for ETS Trains in Malaysia as they can often run full during weekends and holiday periods.
Tickets cost the same as from the station ticket counter, so it saves you a trip out to the station days before your departure to obtain your reservation.
From the station in Butterworth to the ferry terminal, you have to walk for around five minutes.
From the jetty in George Town, you can easily walk into the old town of take a RapidPenang Bus from just in front of the jetty.
Buses run to all parts of the city and around the island to popular destination like Batu Ferringhi Beach.
